Whether you have been newly diagnosed with diabetes or you have been living with it your entire life, you want to have the latest information on the management of your disease. Our education and counseling are offered at two locations – University of Maryland Prince George’s Hospital Center.
The Diabetes Self-Management Program meets the national standards for diabetes self-management education earning recognition by the American Association of Diabetes Educators. Diabetes educators work with you and your provider to create a plan that includes detailed and important information resulting in greater understanding of diabetes and its complications.
A physician referral is necessary for patients taking diabetes education classes. All diabetes management services are offered by appointment only.
The diabetes education program includes:
- Annual follow-up,
- Basic diabetes survival skills,
- Basic meal planning,
- Carbohydrate counting,
- Diabetes self-management classes,
- Insulin medication therapy,
- One-on-one counseling.
With the completion of the full curriculum, follow-up calls to individual participants are made by a diabetes educator at three-month intervals for questions, concerns, and verification of goals met/unmet.
