Transitional Care
Your care is important to us. Our post-discharge care coordination program is designed to coordinate your healthcare needs so that you can continue your recovery at home. We are your ongoing resource for:
- Education about your diagnosis and treatment
- Getting home-delivery of medications
- Scheduling follow-up appointments
- Finding a primary care provider if you don't have one
As part of our commitment to assisting you with transitioning from hospital back home, you may be assigned a Care Coordinator who will be your designated point of contact for your post-discharge support. Your Care Coordinator will be a resource for you should you have any questions about your care and recovery after you have left the hospital.
